Emergency information cards offer a quick, easy way to keep track of allergies and conditions for emergencies. Simply check the boxes next to the preprinted alerts or fill in up to two that are not listed. The reverse side offers three preprinted allergies (penicillin, codeine and sulfa) with boxes to check and up to three lines to fill in other allergies. Below the allergies, preprinted prompts include patient's name, patient's telephone number, name and telephone number of doctor, pharmacy and its telephone number. Fold card to credit-card size to store in a wallet for quick reference. Each self-laminating card is strong, durable and water-resistant. The folded card measures 3-3/8 wide x 2-1/8" high."
